The FBI Vault’s “Frank Cullotta” file, Part 01 (Final), has 59 pages. After a deleted-page information sheet, scan 2 is a Las Vegas airtel of 5/28/80 to the Director, from the Special Agent in Charge, Las Vegas, file 183-618, with the subject, as far as the OCR can be read, “Sherwin Irvin Lisner (deceased), RICO Murder.” The rest of the part is made up of FD-302 forms headed “Surveillance log for surveillance of My Place (MP) and Upper Crust Pizzaria (UCP),” in the “Pioneer Plaza” at Las Vegas, dated June and July 1980, each printed with Las Vegas file number 183-618 and the caution that it “contains neither recommendations nor conclusions of the FBI.”
The logs list, by time, who arrived and left, vehicles, and many unidentified persons described only by sex, race and hair. Names and many lines are redacted. This record does not name the unidentified visitors, and neither does this archive. Frank Cullotta is named repeatedly, with entries such as “Cullotta exits UCP and enters MP.” On scan 18, a log for July 3 to 4, 1980, has the entry “1:35 AM Anthony Spilotro observed through front windows of UCP.” On scan 54 a later log has “Tony Spilotro” entering the Upper Crust with others at 9:50 PM and leaving with Cullotta for My Place at 9:55 PM.
This page was filed from the OCR text of the Vault’s viewer. The page image of scan 18 is now stored here; the rest of the text is quoted as far as the OCR can be read and has not been compared with the other scans. The file states no role or family for Cullotta. The logs place the pizzeria and the bar at the “Pioneer Plaza Shopping Center, Maryland Parkway and Flamingo Road, Las Vegas, Nevada.” The street number of Pioneer Plaza is not given reliably in the OCR and is not stated here.
